School Forms Online (SFO)
Registration Package
Our 2024/2025 School Registration Forms are now available online.
We ask that all families complete their school forms online (SFO) by June 30, 2024.
There are 2 components to the online forms:

School Forms Online (SFO) = all forms pertaining to registration.

Family Access Module (FAM). Once logged in, you can access the Family Access Module (FAM) by clicking it at the top of your screen. FAM allows you to update your information, access the staff & school directory, and view your child’s attendance (once published).

Our 2024/2025 Permission Forms for occasional use are now available to download here.
Please see instructions below:
1. Permission to Administer Medication Form:
If your child requires medication to be administered at school in 2024/2025, please complete this permission form and submit it to Shama in the School Office, along with the medication.
2. Permission to Apply a Non-Prescription Topical Product Form:
If your child requires that we apply a Non-Prescription Topical Product at school in 2024/2025, please complete this permission form and submit it to Shama in the School Office or teacher, along with the product.
